Official UFO Reporting Form

This is the FAB's 19-page official questionnaire for documenting OANI (Objeto Aéreo Não Identificado) reports, structured to capture witness demographics, environmental conditions at the sighting site, object characteristics, motion and trajectory, sound profile, crew/occupant descriptions when applicable, photographic and radar evidence, and physical aftereffects on the location, people, animals, and equipment. The form was digitized from FAB SIOANI originals by Edison Boaventura Jr of GUG (Grupo Ufológico de Guarujá) and represents the intake template that feeds the investigation pipeline routed to the Arquivo Nacional under Ordinance 551/GC3 of 2010. Sections are numbered I through VII and progress from observer biographical data through site description, atmospheric conditions, object physical attributes, occupant descriptions, photographic/radar corroboration, and concluding observer and CIOANI chief signatures. The text is heavily OCR-degraded but the section architecture remains legible.
